【摘要】 目的 了解大环内酯抗生素抗炎效应的细胞学机制。方法 采用内毒素注射制作大鼠急性肺损伤模型,观察大环内酯对肺损伤组织中性粒细胞(PMN)扣押量的影响,并采用原位末端标记DNA技术检测支气管肺泡灌洗液(BALF)中PMN的凋亡,同时比较各类代表性抗生素体外对PMN凋亡的影响。结果 预先给予大环内酯抗生素可显著减少内毒素肺损伤组织中PMN的扣押量,并显著促进BALF中PMN的凋亡。而体外实验显示除大环内酯抗生素外,β-内酰胺类、氨基糖甙类、喹诺酮类的代表药物均未显示有促进PMN凋亡的作用。结论 大环内酯抗生素有明显促进PMN凋亡的作用,此可能为大环内酯抗菌和抗炎双重作用的重要细胞学基础。

Neutrophil Apoptosis Accelerated by Macrolides in Acutely Injured Pulmonary Tissue ZHOU Xiang-dong, HUANG Yong. Dept. of Respiration, Daping Hospital, Third Military Medical University, Chongqing 400042

    【Abstract】 Aim To evaluate the cellular mechanism of the anti-inflammatory action of macrolide. Methods Lipopolysaccharide (LPS) was injected into rats to induce acute lung injury. The effect of macrolide on the quantity of polymorphonuclear neutrophils (PMNs) recruited by acutely injured pulmonary tissue was also observed. The PMN apoptosis in bronchoalveolar lavage fluid (BALF) was examined with in situ end-labelling technique. Effects of several kinds of typical antibiotics on PMN apoptosis were also examined comparatively.Results The quantity of PMNs recruited by the acutely injured pulmonary tissue reduced markedly by the pretreatment with macrolide, and PMN apoptosis in BALF was accelerated obviously by macrolide. The typical agents of β-lactams, aminoglycosides and quinolones didn't show any effect on PMN apoptosis in vitro. Conclusion Macrolide has remarkable effect on accelerating PMN apoptosis. This may be the cellular basis of the double function (antibiotic and anti-inflammation) of macrolide.
